Calling all Completed Math and Reading Stars!

The time has come! All Math and Reading Stars must be completed by Wednesday, April 15

 

When your student has completed all the requirements, 

please complete this form to receive a Math medal, Reading medal or both! 

Note: Forms must be completed by Friday, April 24 

to ensure all medals will be ordered and delivered by the end of the school year.

Each 1st – 5th grade student who earned 24 stars in math or reading (or both!) this year will receive a medal and certificate to recognize their extra work. Remember, each star represents:

• 30 minutes of extra math practice 
• 1 hour of extra reading beyond what is expected for his/her grade level
